I wanted to suprise my girlfriend and purchase her a ticket to come visit. She is overweight and would need to travel with the armrest up and might need an extention (not sure on the extention). I know Southwest & Continental Airlines charge for 2 seats. Which Airlines is the best for a large size person?

Buy her two tickets. Flying her to see you might make her day, but it will ruin the trip for the people who sit near her.

Not only they are thinking about the passengers next to such passenger, but it also can be safety issue in the event of a crash or emergency landing. You should call the airline and get their policy at least in writing so that there is no confusion at the airport.
